I'm joining a zoom conference tomorrow afternoon and, with my wife sitting beside me, we'll be a lot further from the screen - and hence the webcam - than if it was a simple one-to-one call. When people video call using a small device, the very nature of the size of the units means that you will appear to be looking at the little built-in camera, although in actuality you'll almost certainly be looking at the image of the person on the other end of the call. I think the issue with not appearing to look at the caller comes with the use of larger monitors, where the webcam is proportionately further from the centre of the screen than it would be on a phone or tablet. The screen is set at a comfortable eye level and the webcam is mounted to the front of the shelf immediately above the top of the screen (literally an inch above it).
